Dr. Zimmerman, a graduate and faculty member from our New Directions in Diabetes Care Collaborative, will share lessons learned from his experience in quality improvement and demonstrate how to develop a framework for planned care. Other topic areas in the works include, partnering with patients on self management of chronic conditions, completing SAMs, and developing practice improvement projects, just to name a few.
Learning Objectives:
- Define the relationship between quality improvement and practice re-design.
- Develop greater understanding of STOCs (Small Tests of Change) and how to implement them in medical practice.
- Employ at least three practice pearls within the first two months post-assembly.

Did you know that there is a significantly higher rate of diabetic complications in your Hispanic patient population? For example, Hispanics have higher A1Cs, are more likely to have retinopathy, lower extremity amputation, and cardiovascular mortality than non-Hispanic whites. Hispanic and African American patients are also less likely to welcome introduction of insulin for diabetes care. Culture, and how you address beliefs, can make a difference in the clinical improvements in your ethnic patient populations. The good news is that there are easy and practical approaches to employ in your practice.
Learning Objectives:
- Identify several cultural beliefs that impact how many Hispanics understand the etiology of Diabetes.
- Identify some herbal remedies that may be used by their Latino Diabetic patients and clinical efficacy of these remedies.
- Gain an understanding of the communication required for an effective cross-medical encounter with a Hispanic patients
- Understand how to deliver cultural competent care using the LEARN framework.

Self-Assessment Modules (SAMs) and Patient Simulation (View/Download PDF)
This session will assist family physicians with navigating Part II of the SAMs with faculty who have completed several. For those who have completed the Part I learning and assessment activity, this session will prepare you for the patient simulation component. This component is where your knowledge is put into practice. The patient simulation component consists of a clinical case that is representative of the disease state. The patient's condition evolves as the simulation progresses, and the physician must respond. The simulation continues until either the patient's condition is stable for 3 successive office visits or 2 years of simulated time has elapsed, whichever comes first.
Learning Objectives:
- Review the MC-FP process, in particular the SAM process for the Patient Simulation Module.
- Discuss the issues presented in the hypertension and diabetes SAM Patient Simulation modules, using group interaction and cases.
- Complete an online SAM after returning from the ASA
