Just What is PCMH?
CAFP produced a series of videos, with family physicians and Academy leaders, to address topics as wide ranging as Medicare SRG, payment reform, and the Patient Centered Medical Home. This video explains the concept of the medical home and why it should be the foundation of any serious health care reform efforts. Family physicians Carla Kakutani, Jeff Luther, Candace Lawson and Taejoon Ahn describe the tenets of PCMH, and key legislators talk about their views of the family physician led medical home.
