Humboldt County, participating in the Robert Wood Johnson Foundation’s “Aligning Forces for Quality” program, has developed the Community Health Alliance of Humboldt-Del Norte, Inc.. The Community Health Alliance’s goals are to provide a high-quality, integrated, patient-centered system of care that is accessible to all North Coast residents. The county’s program has been identified as one of California’s leading PCMH developments.
The University of California, Davis (UC Davis) has become one of the first residency programs in California to incorporate PCMH-based education into their residency training. In addition to traditional didactic sessions and clinical work, residents are exposed to a wide array of innovative practices designed to boost care coordination, improve quality and increase access for patients. Residents are trained to use EHRs to track quality indicators and patient satisfaction data, to coordinate care for patients with newly diagnosed mental illness, and to participate in “practice huddles” to improve teamwork and communication in the clinic. Patients also benefit from this initiative as the program utilizes a patient portal, same day appointment scheduling and registries for the management of chronic conditions. UC Davis has just applied to the NCQA for recognition as a Physician Practice Connections certified PCMH. The California Association of Public Hospitals and Health Systems is working to develop medical homes in primary care clinics as part of California’s Healthcare Coverage Initiative.
The California Department of Health Care Services (DHCS) has piloted several initiatives to better coordinate care for approximately 360,000 seniors and persons with disabilities (SPD) in Medi-Cal’s fee-for-service program.

Sonoma County’s Health Action, a county run council with a mission to improve community health, has included among its projects a Medical Home Collaborative. Its purpose is to create a learning community of primary care providers who will work collaboratively to transform their own practices and work to support implementation and dissemination of the medical home throughout Sonoma County. This project will build local capacity and resources to implement the medical home model and lays the foundation for the goal of connecting all residents in Sonoma County with prevention-focused primary care and community resources to support healthy living.
