This Resource Center provides everything a family physician needs to consider a move to the PCMH model.  Compelling studies show how providing care in a PCMH improves patient outcomes while reducing unnecessary health care costs. ‘How To’ and best practices information in print, electronic, audio and video formats explain the concrete steps required.

This how-to guide was developed by the Agency for Healthcare Research and Quality to support primary care transformation. The guide was designed to build the internal capacity of a practice and support incremental and transformative changes to improve patient outcomes.

Patient Centered Medical Home
PCMH is a model of care that integrates patients as active participants in their own health and well-being. Patients are cared for by a primary care physician who leads the medical team that coordinates all aspects of preventive, acute and chronic needs of patients using the best available evidence and appropriate technology to improve quality in a cost effective manner. The value of PCMH services is recognized by enhanced payment.
