National Health Care Reform and the PCMH

The Patient Protection and Affordable Care Act (PPACA) contains provisions that require the Centers for Medicare and Medicaid Services (CMS) to sponsor demonstration projects or pilots, give states the option to provide “health homes” for enrollees with chronic conditions, provide incentives for the development of Accountable Care Organizations (ACOs), establish an Innovation Center with the purpose of developing innovative payment and delivery arrangements and provide support for the training and payment of primary care providers.

CMS currently has three medical home demonstrations in development. CMS is in the process of implementing the Multi-payer Advanced Primary Care Practice Demonstration (for which the California state government did not apply), and will be developing the Federally Qualified Health Center Advanced Primary Care Practice Demonstration and the Medicare Medical Home Demonstration.
