CAFP's PCMH Advocacy Efforts
Framing the PCMH discussion in the key areas of CAFP business will facilitate transformation of family medicine practices toward comprehensive health care reform.
Sponsor State Legislation
Last legislative session, CAFP sponsored a bill (AB 1542) to define the PCMH based on the Joint Principles. The bill made it far in the legislative process, but got caught up in the politics surrounding the state budget agreement. CAFP hopes that the diverse coalition that supported the bill will once again join forces to advance the bill when CAFP reintroduces it early next year.
Medicaid Section 1115 Waiver
CAFP
has also advocated for the medical home in non-sponsored legislation. For
example, the goal of the new Medicaid Section 1115 Waiver (as detailed in SB
208) is to improve care management and coordination in public health
programs for seniors, persons with disabilities, dual eligible patients
(Medi-Cal and Medicare), children with special health care needs and persons
with behavioral health conditions. SB 208 authorizes the state government to
seek federal authority and funds through the Waiver and to enroll beneficiaries
into specific organized delivery systems, such as managed care, enhanced
primary care case management, or a medical home. CAFP will continue to be
actively involved in the process to ensure that the new waiver reflects the
true value of the PCMH. See how California proposes to advance these program changes.
Meetings with Legislators, Opinion-Makers, Payors and Business
CAFP has developed an excellent working relationship with Paul Grundy,
MD, MPH, IBM’s Global Director for Health Care Transformation. Dr.
Grundy and various CAFP leaders and executive staff members have had
several days of meeting in Sacramento, San Francisco and Fresno
addressing PCMH.
