For patients, consumers, professionals and policymakers, AHRQ is an excellent resource for evidence-based medicine and health research. AHRQ has a toolkit to help physicians’ offices, clinics and ambulatory care facilities improve their testing processes.

 

American Academy of Family Physicians

AAFP’s Patient Centered Medical Home section offers a starting point for PCMH. TransforMED also offers free resources for those who sign up for their online network, Delta Exchange.

 

American Academy of Pediatrics

AAP’s National Center for Medical Home Implementation has resources for health professionals, families, and anyone interested in creating a medical home.

 

American College of Physicians

ACP’s PCMH center includes tools and resources for physicians engaging in PCMH Transformation.

 

Center for Excellence in Primary Care

A collaboration between the Department of Family & Community Medicine, University of California, San Francisco, and The Permanente Medical Group, the Center’s mission is to transform primary care at the regional, national and international level.

 

Improving Chronic Illness Care

ICIC’s robust PCMH resource center is dedicated to improving care through implementation of the Chronic Care Model. Tools include change concept workbooks, curriculum and training materials, PCMH literature and more.

 

Institute for Healthcare Improvement

IHI’s Knowledge Center offers tools, change ideas and measures to guide members to the PCMH. White papers, audio, video, improvement stories and other resources are available for practice facilitation.

 

Joint Commission Primary Care Medical Home

The Joint Commission certifies PCMH in California.

 

National Committee for Quality Assurance

The NCQA Certification Program reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP), and American Osteopathic Association (AOA). NCQA 2014 PCMH Standards are available here.

 

National Quality Forum

NQF works to catalyze improvement in health care delivery. Their website has information on their measures, reports, and tools.

 

Patient Centered Primary Care Collaborative

Safety Net Medical Home Initiative

The Safety Net Medical Home Initiative is a national PCMH demonstration to move 65 primary care safety net sites to the medical home model.

 

Utilization Review Accreditation Commission

URAC developed a PCMH program to educate and guide health care practices, sponsoring health plans, insurers, and pilot programs, on how to transform practices into PCMH.
