Subscribe to the Academy in Action In June, Lumetra hosted its first Care Transitions Conference, the focus of which was “Finding Solutions, Improving Care Coordination” and it brought together health care professionals and leaders from hospitals, medical groups, and provider and consumer organizations to discuss challenges related to care coordination and to share best practices and innovative ideas.
