You are correct; multiple demonstration projects around the country have demonstrated that the patient centered medical home (PCMH) model can be both cost- and quality-effective, but must be supported appropriately by payers, whether public or private. The Patient Centered Primary Care Collaborative, based in Washington, D.C., is a major advocate for payment reform to support the PCMH, as are both CAFP and AAFP. A new Medicare Multi-Payer PCMH Demonstration Project will pay $10/per month for Medicare patients in six states if other payers participate as well. The trend for supporting PCMH is heading in the right direction, but we're not there yet.

medical homes and reform

How does this manner of care effect the health insurance realities that we are currently dealing with? From this short video I can see where these institutions might really provide some vital coverage and care for certain members of our communities, I'm just always curious as to how likely it is that our Health Insurance programs will respect ideas like this one. I think that this is really a more efficient system in many ways, but there are many things like this that are, counter-intuitively made impossible by our current systems.
