CAFP is surveying members about the Patient Centered Medical (PCMH). Our goal is to have comprehensive data on where CAFP members see themselves on the PCMH adoption continuum. In addition, we'd like to know what tools and resources we can provide that would be of greatest valuable to our members.  We know members have a range of views on the concept of the medical home, as well as many PCMH activities and functions (i.e., improving patient access and communication). Understanding these views will help shape the development of programs and resources.
