New Directions in Diabetes Care

CAFP has been searching for ways to offer its members a user-friendly, time-efficient, and flexible program to address clinical excellence and practice re-design. We have launched a new initiative called New Directions in Diabetes Care to help physicians optimize the care of patients with diabetes in office practice. This initiative is both in keeping with, and builds upon, “New Directions in Primary Care” outlined in the AAFPs’ Future of Family Medicine and the Institute of Medicine’s Crossing the Quality Chasm, as well as implements the goals of CAFP’s Strategic Plan.
Diabetes Collaboratory
The Diabetes Collaboratory is one part of New Directions in Diabetes Care. The Web-based collaboratory is a virtual collaborative fashioned on the practice improvement collaborative model used successfully by the Institute for Healthcare Improvement for more than 10 years. Care Model
The New Directions in Diabetes Care Collaboratory is based on the Chronic Care Model, developed by Improving Chronic Illness Care, a national program of the Robert Wood Johnson Foundation. Our particular change package was developed by Lumetra. Click here for the NDDC Care Model Change Package.
