Diabetes Collaboratory


CAFP’s New Directions in Diabetes Care Collaboratory provides ready access to the most current information about best practices for the care of patients with diabetes, tools for practice re-design, and the use of information technology in the management of patients with diabetes. Participants engage in a series of online learning sessions, undertake small tests of change in their practices using the Plan-Do-Study-Act model, and then report back to the Collaboratory. The entire program, developed in the form of modules, enables participants to adopt change incrementally to minimize disruption, and to increase the likelihood of buy-in from all members of their teams.

Each of our one-year collaboratories consists of family physician practices from around the state. Typically, we have between 12-20 teams per collaboratory, and some teams are led by staff of a family physician member.

STOC Reporting
Small tests of change (STOCs) are a process used in quality improvement. They are also known as the Plan-Do-Study-Act cycle. STOCs are changes in your office practice (i.e., try a flow sheet), not a task (i.e., draft a flow sheet). They are based on the goals set by you and your team as part of the NDDC Collaboratory. Report your small test of change using this easy form.

ceCity
NDDC Collaboratory participants can use the ceCity platform to enter their practice profile, assessment and data from chart pulls, see a report on data compared to other teams, and select action plans.
