CAROL HAVENS, MD

Carol Havens, MD became the second PMG president in as many years. 

Dr. Havens is director of clinical education for physicians in Kaiser Permanente's Northern California Region and a staff physician in the Chemical Dependency and Recovery Program at Kaiser Permanente in Sacramento.  She is a dedicated and highly effective physician leader who has served on several CAFP committees and task forces, and believes that the stronger the Academy's voice, the stronger the influence family physicians can have in the health policy debates that shape California's, and the nation's, health care future.

 

Jack Chou, MD

Jack Chou, MD

At 38 years old, Jack Chou, MD became the youngest president in CAFP's 62-year history. He is also the Academy's first Chinese-American president as well as the first Permanente Medical Group president. Dr. Chou is also active with the California Medical Association and the Los Angeles County Medical Association.

He is a proud husband and father to twin daughters Sarah and Samantha. His first son, Nathan, was born on February 22, 2010.

 

Carla Kakutani, MD, FAAFP

Carla Kakutani, MD has been in private practice with a multi-specialty group based in Davis, California since 1993. She practices full-spectrum family medicine, including obstetrics and managing hospitalized patients. She received her Bachelor of Science degree from the University of California, Davis, attended medical school at the University of California, Irvine, and completed a family medicine residency at Stanislaus Medical Center in Modesto, California in 1992.

Dr. Kakutani is a CAFP past president and and is the Chair of CAFP’s delegation to the California Medical Association, as well as the Family Physician Political Action Committee (FP-PAC). She is also a member of the American Academy of Family Physicians’ Commission on Governmental Advocacy.

Despite all of the above, she is still happily married to her husband, Mike, with son Thomas and daughter Rosie.


Susan Hogeland, CAE

Susan Hogeland, CAE, has been the CAFP Executive Vice President since 1991.  Prior to that, she was Assistant Executive Director of the San Francisco Medical Society for 11 years.

Ms. Hogeland holds a Master of Arts in Political Science from San Francisco State University, and obtained her undergraduate degree in Political Science from the University of Maryland, College Park, while working on Capitol Hill.

She is a 1989 graduate of the Institute for Organization Management and received her  Certified Association Executive designation in 1995.  She was named Association Executive of the Year by the Northern California Society of Association Executives in 1997, received the Award of Merit of the American Academy of Family Physicians in 1999, and the AAFP President’s Award in 2005.

Ms. Hogeland has served in a number of volunteer capacities, including Board member of the Northern California Society of Association Executives, Treasurer of the Health Care Leadership Institute, Treasurer of the California Primary Care Consortium, member of the Advisory Board of the National Youth Leadership Forum, Member and former chair, UCSF School of Dentistry Board of Counselors. She is active in the American Association of Medical Society Executives, and has served as co-chair of Membership Development, a member of the Council on Collaboration, and the Health Care Policy Committee.  She has served on the Government and Legislative Affairs and Chapter Affairs Committees of the American Academy of Family Physicians.
